• Provide the required five (5) hours of annual dementia (a decline in memory, reasoning, and thinking skills that is severe enough to interfere with daily life) specific in-service training (professional development and educational activities) to Certified Nursing Assistants (CNAs) when dementia specific in services training reviewed from January 2025 through May 2026 showed the facility only provided two hours of dementia-specific training
• Ensure a resident received medication in accordance with facility's policy and procedures (P&P)
• Ensure a resident received the required assessments and monitoring as outlined in the facility’s policy and procedure (P&P) for “Use of Restraints”
• Ensure visual privacy was provided to a resident
